For Infant & School Age Immunizations:

For COVID and/or FLU vaccines only, book at Get vaccinated
If your child was vaccinated elsewhere, send their immunization record (in English) to mychildsimmshistoryVC@vch.ca or bring it in a week before your appointment
Book using your child’s legal name and Personal Health Number (PHN). If no PHN, type “none”.
Please arrive 15 minutes early. If you arrive late we may need to reschedule your appointment.
Book one slot per child.
For siblings or twins, book appointments back-to-back.
If your child needs special support, call your local health centre.
