Update: School Age Appointments (Kindergarten to Grade 12)

• For appointments before June 28, book here

• For appointments on or after June 28, book on this page.

• Infant (0-3 years) appointments stay the same.

For Infant & School Age Immunizations: 1. For COVID and/or FLU vaccines only, book at Get vaccinated

1. If your child was vaccinated elsewhere, send their immunization record (in English) to mychildsimmshistoryVC@vch.ca or bring it in a week before your appointment

2. Book using your child’s legal name and Personal Health Number (PHN). If no PHN, type “none”.

3. Please arrive 15 minutes early.

4. Book one slot per child.

5. For siblings or twins, book appointments back-to-back.

6. If your child needs special support, call your local health centre.
